Support Coordination & Recovery Coaching

Our Psychosocial Recovery Coaching and Support Coordination services are designed to empower you on your journey to recovery and independence. We work closely with NDIS participants to build capacity, leverage strengths, and foster resilience in a supportive, collaborative environment. Jasmine and her team assist you in understanding and navigating your NDIS plan, connecting you with the right services, and ensuring you have the tools and guidance to thrive. Whether it's designing personalized recovery plans or coordinating support, our goal is to help you achieve your goals with confidence and ease.

Functional Capacity Assessments

Functional Capacity Assessments are crucial in determining your ability to perform daily activities and identifying areas where you may need additional support. Our team conducts thorough assessments to evaluate your functional abilities, providing detailed reports that inform your NDIS plan and help you access the necessary services. We approach each assessment with sensitivity and professionalism, ensuring that your needs are accurately reflected and addressed.

Support Work

Our Support Workers provide hands-on assistance with daily activities, ensuring you have the practical support you need to live independently and comfortably. Whether it's help with personal care, household tasks, or community participation, our team is here to assist you in achieving your goals. We take a person-centered approach, tailoring our support to meet your specific needs and preferences, so you can live your life to the fullest.

Mental Health Social Work

Disability social workers play a crucial role within the NDIS, providing comprehensive support to individuals with disabilities, their families, and caregivers. They collaborate closely with participants to develop tailored support plans that focus on achieving personal goals. As strong advocates, they ensure that participants receive the necessary services and that their rights are protected within the NDIS framework. Beyond advocacy, social workers offer counseling and emotional support, helping individuals and families navigate the emotional challenges of living with a disability. They also leverage their extensive networks to connect participants with appropriate service providers, from therapists to community groups. Through education and training, they empower individuals and families with the knowledge and skills needed to become more self-sufficient and active in their communities. In challenging situations, social workers provide immediate crisis support and connect participants with essential resources. They are dedicated to promoting social inclusion by combating stigma and discrimination, striving to create a more inclusive society where everyone can thrive. Despite the challenges of navigating complex NDIS systems and working within resource constraints, their commitment ensures that participants receive the support they need. Social work services can be accessed through NDIS Plans under specific line items, making their essential work both accessible and impactful.
